1. Completed and signed application form
2. A certified translated copy of your certification of secondary education completion (If you have not completed high school, you may be given consideration for acceptance on a case-by-case basis).
3. Proof of financial responsibility:
   1. Signed Certification of Finances Form.
   2. Financial statement from one of the following:
      • Personal Funds. A translated financial statement on official bank letterhead signed by a bank official showing a current bank balance.
      • Funds from relatives or another personal sponsor. A translated financial statement on official bank letterhead signed by a bank official showing a current bank balance.
      • Government or private sponsor. A translated signed copy of your award from the sponsoring agency.

• You can complete it online or download the application form (requires Adobe Reader), print it out, complete it, and mail or fax it to us.
• You can write, e-mail, call, or fax us and we will send you an application packet.

When your application is complete, make sure to:

• Sign it
• Enclose the $50 deposit and $50 application fee (total:100). The $50 application fee is non-refundable. $50 deposit fee will be applied to final bill upon enrollment. Checks or money orders (in U.S. dollars) should be made to Youngstown State University.
• Complete the Certification of Finances form and sign it. Have the sponsor (relative, embassy, company, etc.) sign it.
• Submit a signed and certified financial statement (in U.S. dollars) from the sponsor's bank, dated within six months of enrollment.
• Enclose certified copy of proof of secondary school completion.

After the ELI receives all application documents, and the student is admitted to the ELI, a Form I-20 will be sent to the student if all support documents are complete.
