 Scholars are required to document their community service endeavors as well as complete an end of the year evaluation form detailing total honors hours, service hours and other program related information. You can find all of the necessary scholar forms to the right.  |
Each University Scholar is required to complete a form detailing
their community service progress. Use the printable form below to submit
each activitity you complete. This form is only necessary for events that were not coordinated through the Honors Office. Other Forms Contract Honors and Senior Thesis forms can be found here. |
Remember
this information is necessary as it is used to evaluate your continuation
in the program. So, please take the time to submit your information.
