	Scholars are required to document their community service endeavors as well as complete an end of the year evaluation form detailing total honors hours, service hours and other program related information. You can find all of the necessary scholar forms to the right. Use the printable form below to submit each activitity you complete. This form is only necessary for events that were not coordinated through the Honors Office.
