Club Sport Registration Information

Registration for Club Sports occurs annually and forms should be submitted in May for the next academic year. The following forms are required to officially register your Club Sport with Campus Recreation (as a Club Sport) as well as the Student Programming Office (as a Student Organization).

Club Sport Application (Campus Recreation) (PDF)
Complete this form when registereing your Club Sport with Campus Recreation. This application must also be turned in with the Student Organizaiton Registration Form in order to officially register the group with Campus Recreation and the Student Programming Office.

This application must include:
1. Copy of the constitution and by-laws, including a statement of purpose.
2. A list of members, including Banner ID numbers.
3. A proposed schedule of competition or program activities.
4. A proposed budget.
5. YSU E-Mail address required.

Student Organization Registration Form (PDF)
Complete this form when registering a new student organization or when re-registering for the current academic year. Each organization is required to re-register each academic year in May or as soon as they come back to campus in the Fall.

IF YOUR OFFICERS CHANGE MID-SEMESTER:

Changing of the Guard Form (PDF)
Complete this form if your organization officers change during the mid-semester or at the beginning of Spring term. Make sure your advisor signs off on these changes and then submit the form to the Student Programming Office.
